Call (888) 712-5839 or email medical@aaraamhealth.com with subject “Adverse Event Report,” and include: (a) your name and contact information; (b) product name, lot/lot code, and expiration (if available); (c) dosage and timing; (d) description of symptoms and onset timing; (e) any medical care received (e.g., ER, hospitalization); (f) relevant medical history and current medications/supplements; and (g) photos or documents if available.

7.3.    Use of information

7.3.1. By submitting a report, you authorize Aaraam to use and disclose information for safety monitoring, investigation, and reporting to regulators, as permitted by law. We may follow up to obtain additional details for a thorough assessment.

8.     Privacy Reference

9.     Accessibility
